Technical Assistance:
Technical assistance was provided regarding documentation of proof of identification.

Comments:
An unannounced monitoring inspection was conducted today from 9:05am-11:00am. There were 13 children directly supervised by 4 staff and all classrooms were in ratio. The physical plant, programming, 4 staff records, 5 children?s records, 2 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed reading books, doing puzzles, engaging in circle times, singing and dancing, and practicing writing. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children. The center was clean and organized. Standard #: 22VAC40-185-60-A
Description: Based on record review, 1 of 5 children's records was missing 2 complete emergency contact addresses. Evidence: 1. Child #2 (Start date: 6/4/19)'s record was missing 2 complete emergency contact addresses.

Plan of Correction: We will email mom and dad today and hopefully get a response this afternoon. We will let them know we need the information by drop off tomorrow.

Standard #: 22VAC40-185-280-B
Description: Based on observation of the physical plant, a hazardous substance such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. In the Toddler classroom, the door to the bathroom was ajar and unlocked. A spray bottle of cleaning material was located on the back of the toilet. 2. The door was not locked and makes the area accessible to the children in care.

Plan of Correction: We will lock all cleaning supplies.

Standard #: 22VAC40-185-550-M
Description: Based on record review, the center has not maintained a complete record of children's serious or minor injuries. Evidence: 1. Child #1's record contained 7 injury reports and all were missing the future action to prevent recurrence of the incident.

Plan of Correction: On Child #1's injury reports and future children's injury reports, we will ensure the future action to prevent recurrence is completed.
