Comments:
The licensing inspector conducted an unannounced monitoring inspection Monday, June 10, 2019 from 10:05am to 12:50pm. The infants were observed playing on the floor and sleeping. The older infants and toddlers were observed getting changed, playing on riding toys and singing songs. All classrooms were inspected today. Due to the rain the playground was not checked. Five children?s records and seven new employee records were reviewed during this inspection. The center administers medication, however none is being administered at this time. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 05/22/2019 Shelter-in-place drills: 04/03/2019 Total enrollment today: 23 Today, the following child to staff ratios were observed: Infant I (6 weeks to 12 months) ?9:4 Infant II (12-14 months) ?1:1 Toddler I (14-18 months)-5:2 Toddler II (18-24 months)- 8:3 School Age ?not present The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-130-B
Description: Based on a review of children's records on 06/04/2019, the center did not ensure that documentation of additional immunizations was obtained once every six months for children under the age of two years. EVIDENCE: 1. The records of Child #2, who is two years old as of last week, but has been enrolled at the center since 06/04/2018, and Child #3, who is 21 months old, did not contain documentation of immunizations every six months.

Plan of Correction: Will get parents to bring in updated immunizations.

Standard #: 22VAC40-185-140-A
Description: Based on a review of children's records on 06/10/2019, the center did not ensure that Each child shall have a physical examination before the child's attendance; or within one month after attendance. EVIDENCE: The physical in the record of Child #1, who started at he center on 06/14/2018, is dated 04/25/2019.

Plan of Correction: Going forward will be more observant of dates on physicals.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records on 06/10/2019, the center did not ensure that two of the five records reviewed contained the required information. EVIDENCE: 1. The most recent documentation of a review of the Child #2's emergency contact information by the child's parent was when the child started at the center on 06/04/2018. 2. The most recent documentation of a review of the Child #3's emergency contact information by the child's parent was when the child started at the center on 11/01/2017.

Plan of Correction: 1. Will have the parent of Child #2 review this information today. 2. Will have the parent of Child #3 do review when they return at the end of summer break.

Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records on 06/10/2019, the center did not ensure that each staff record contained the address of a person to be notified in an emergency which shall be kept at the center. EVIDENCE: The records of Staff #5 and Staff #7, did not contain a full address for the emergency contact listed. Each record did not contain the house number or street name.

Plan of Correction: Corrected on site. Staff provided missing information. Will double check records going forward.

Standard #: 22VAC40-185-540-C
Description: Based on observation on 06/10/2019, the center did not ensure that the required first aid kit contained the minimum items. EVIDENCE: The first aid kit for the center did not contain adhesive tape.

Plan of Correction: Will get it by today.
