Comments:
The licensing inspector conducted an unannounced monitoring inspection on Wednesday, May 29, 2019 from 11:05am to 02:45pm. The children were observed playing on the playground, working in small groups and eating lunch. Lunch consisted of turkey and cheese wraps, cucumbers with ranch dressing, peaches, and milk. All classrooms were inspected today as well as the facility?s mini buses and playgrounds. 10 children?s records and 10 employee records were reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 05/16/2019 Last shelter-in-place drill: 03/13/2019 The following child to staff ratio was observed: Infants-11:3 Infant II-4:1 Toddlers-10:2 Two's-22:3 Preschool-26:3 Pre-K-27:3 School Age-not present If you have any questions about this inspection, please contact the licensing inspector, Roshon Casey, at (804) 662-9760.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on a a review of children's records on 05/29/2019, the center did not ensure that documentation of additional immunizations was obtained once every six months for children under the age of two years. EVIDENCE: 1. The record of Child #1, who turned two years old this month, and has been enrolled at the center since 07/17/2017, did not contain documentation of immunizations every six months. 2. The record of Child #2, who turned two years old in January, and has been enrolled at the center since 04/17/2017, did not contain documentation of immunizations every six months. 3. The record of Child #6, who turned two years old in April, and has been enrolled at the center since 07/02/2018, did not contain documentation of immunizations every six months.

Plan of Correction: We have done an audit and sent two e-mails to families requesting updated immunizations. Families who have not responded will be contacted individually and asked to update immunizations. We have a tickler system to maintain up to date immunizations for children under the age of two years old and between the ages of four and six years old.

Standard #: 22VAC40-185-160-C
Description: Based on a review of staff records on 05/29/2019, the center did not ensure that staff submit the results of a follow-up tuberculosis(tb) screening every 2 years. EVIDENCE: The most recent tb results in the record of Staff #2 is dated 02/11/2017.

Plan of Correction: Staff will be sent to have tb administered today and read on Friday. Will conduct a file audit on a quarterly basis to ensure that all files are up to date.

Standard #: 22VAC40-185-60-A-7
Description: Based on a review of children's records on 05/29/2019, the center did not ensure that each child's record contained information regarding allergies and intolerance to food, medication, or any other substances, and actions to take in an emergency situation. EVIDENCE: The records of Child #1, Child #8 and Child #10 did not contain information regarding allergies.

Plan of Correction: Parents will be contacted today to stop at the front desk to complete allergy section of registration form. Going forward new families will be asked to complete each section of the VDSS form before enrolling their child.

Standard #: 22VAC40-185-520-A
Description: Based on observation on 05/29/2019, the center did not ensure that over-the-counter skin products shall not be kept or used beyond the expiration date of the product. EVIDENCE: 1. Expired sunscreen that belonged to Child #11 was observed in the Two's classroom. The sunscreen expired 05/2018. 2. Expired sunscreen that belonged to Child #12 was observed in the Infant II classroom. The sunscreen expired 04/2019.

Plan of Correction: Both parents contacted today and asked to bring in new ointments. Teachers will be asked to check expiration dates. Will add reminder to outlook calender to remind teachers monthly to check over the counter products expiration dates.

Standard #: 22VAC40-185-560-F-4
Description: Based on observation on 05/29/2019, the center did not ensure that children three years of age or younger may not be offered foods that are considered to be potential choking hazards. EVIDENCE: The children in the toddler and two's classrooms were served raw cucumbers slices with their lunch.

Plan of Correction: Effective immediately, cooked vegetables will be served as an alternative to raw vegetables to two year olds and younger.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on a review of staff records on 05/29/2019, the center did not ensure that staff obtained a fingerprint based national background check by the required 09/30/2018 time frame. EVIDENCE: The determination letter for the fingerprint based background checks in the record of Staff #4 is dated 02/25/2019.

Plan of Correction: At this point all employees have been fingerprinted, and going forward all employees will get fingerprinted before their first day of employment.
