Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted from 10:00 a.m. - 12:30 p.m. There were 136 children in care being supervised by 22 staff members. Ten children's records were reviewed and in compliance. Staff training records were reviewed for completion of CPR, First Aid, and Virginia Pre-Service Training. An allergy list was reviewed for children enrolled in the program with diagnosed food allergies. Allergy care plans were reviewed for children with diagnosed food allergies. Standard #: 22VAC40-665-520-B-11
Description: Based on review of the list of diagnosed food allergies and children's records it was determined that not all children with diagnosed food allergies had documentation of an allergy care plan on file. Evidence: The written allergy care plans on file for child A and child B were not signed by a physician.

Plan of Correction: Both parents changed students to intolerance.
