Arrived 1:35pm Departed 3: pm A Subsidy Health and Safety inspection was conducted on 6/12/2019 to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are listed on the violation notice. Standard #: 22VAC40-665-540-A Description: Based on the review office five staff files, the facility failed to ensure that all staff complete a TB test or screening before coming into contact with children. Evidence: 1. There are no TB test results on file for staff #1, 4 and 6. Plan of Correction: The director will ensure that all staff complete a TB test or screening before coming into contact with children.
Standard #: 22VAC40-665-580-B Description: Based on the review of five staff records, the facility failed to ensure that all staff have completed the Va Preservice Subsidy training. Evidence: 1. There is no record of preservice training on file for staff #2. Plan of Correction: The director will ensure that all staff complete the training and print the certificate for the records.
Standard #: 22VAC40-665-780-A-3 Description: Based on a review of the emergency drills log and an interview with the director, the facility failed to conduct all required drills. Evidence: 1. There has been no documentation of a lockdown emergency drill. Plan of Correction: The director will ensure that a lockdown drill is conducted this week.
