Technical Assistance:
The renewal process was discussed and the director advised she would be submitting the renewal application soon.

Comments:
An unannounced monitoring inspection was conducted today from 9:45am-12:50pm. There were 40 children directly supervised by 10 staff and all classrooms were in ratio. The physical plant, programming, menu, 5 staff records, 5 children?s records, 3 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed engaging in free play, painting suns onto paper plates, doing a handprint art project, playing on the computer, learning to count, and playing outside on the playground. Infant care was observed and infants were feeding, sleeping, and playing. Positive interactions between children and staff were observed during this inspection. Lunch served today was meatballs in brown gravy, mashed potatoes, cooked carrots, mixed fruit, and milk. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A
Description: Based on review of 5 children's records, 2 records did not contain all required information. Evidence: 1. Child #3's record was missing mother's employer and an address for 1 of 2 emergency contacts. 2. Child #4's record was missing mother's employer and work telephone number, father's work telephone number, and documentation that proof of identification was verified within 7 days of child's first day of attendance. Child #4's first day at the center was 4/29/19.

Plan of Correction: All information will be obtained by today.

Standard #: 22VAC40-185-550-M
Description: Based on review of 5 injury reports, the center has not maintained a complete written record of children's serious and minor injuries. Evidence: 1. On injury reports dated 4/30/19 and 5/15/19 the time of parent notification and future action to prevent recurrence of injury was not completed. 2. On an injury report dated 6/6/19 the time of parent notification was not complete.

Plan of Correction: We will retrain staff on proper completion of accident reports by Friday.
