An unannounced monitoring inspection was completed on June 11, 2019 from approximately 2:30p.m. to 3:45p.m. Twenty-four children were in care and five staff were on premises during the inspection. Staff and children were observed in the indoor space and outside engaged in activities to include arrival and departure from the program, outside play, nap time, the toileting procedures, hand washing and snack time. Interviews and interactions were conducted and medications were reviewed. Five children?s records were reviewed. The staff records will be reviewed at a later date at an alternative center site where they are maintained. The owner/director was available for the inspection and present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Standard #: 22VAC40-185-440-B Description: Based on observation and interview, the licensee failed to ensure the rest mats are identified for use by a specific child. Evidence: 1. Upon arrival, four children were observed during nap time resting on cushions from the furniture in the space. 2. In an interview, staff indicated that the cushions are not identified for specific use by the same child daily. 3. Administrator #1 acknowledged there is not process in which the same cushion is identified for use by the same child. Administrator #1 advised the use of these cushions is temporary and limited to a two week camp after which there will by no use for rest mats. Plan of Correction: The lead teacher will assign identified rest mats to the same child for use daily.
