Comments:

An unannounced monitoring inspection was completed on June 12, 2019 from approximately 2:30p.m. to 4:15p.m. Seventeen children were in care and five staff were on premises during the inspection. Staff and children were observed in classrooms engaged in activities to include arrival and departure from the program, hand washing, free play and playing electronics. Interviews and interactions were conducted and no medications are given. Five children?s records and five staff records were reviewed. The director was available for the inspection and present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Standard #:	22VAC40-185-70-A
Description:	Based on a review of staff records and interview, the licensee failed to maintain all the elements of staff?s records for one of five staff. Evidence: 1. Staff #2's record, hired on September 10, 2018 as a program lead lacked documented experience required for the job position. 2. Administrator #1 acknowledged this information was not documented in this staff record and advised Staff #2 is qualified from previous employment at this organization and was rehired.
Plan of Correction:	This was corrected on site. The staff provided documentation of previous experience qualifying her for the position.
Standard #:	22VAC40-185-550-M
Description:	Based on a review of records, the licensee failed to maintain all required elements of a written record of children's serious and minor injuries in which entries are made the day of occurrence. Evidence: 1. Injury reports from the beginning of the year were reviewed. The injury reports lacked the time and method for how parents were notified of the injury. 3. Administrator #1 acknowledged these elements were not being completed by staff.
Plan of Correction:	The Assistant Director will retrain staff on the injury report requirements.
Standard #:	22VAC40-185-580-C
Description:	Based on observation and interview, the licensee failed to ensure there were emergency numbers posted and documentation containing local emergency information that pertains to each site frequently visited or of routes frequently driven by the center located in the center's bus used to transport children. Evidence: 1. Upon observation of the center?s bus used to transport children, there were no emergency numbers posted or documentation containing local emergency information that pertains to each site frequently visited or of routes frequently driven by the center located in the bus. 2. Administrator #1 acknowledged these items were not on the bus.
Plan of Correction:	The Assistant Director will prepare these documents and place them on the bus.
