06/04/19 Arrived 2:40pm Departed 5:50pm An unannounced monitoring inspection was conducted today. Seventy-three children were present with six staff. The children were doing an art project, reading, playing games, eating a snack, playing in the gym and playing outdoors. Previous violations, medication, staff and children's files were reviewed. Standard #: 22VAC40-185-70-A
Description: Based on the review of nine staff files, the facility failed to ensure that all staff files contain all required documents. Evidence: 1. There are no references on file for staff #4, 5, 7 and 9.

Standard #: 22VAC40-185-550-D
Description: Based on a review of the emergency escape drill records the facility failed to conduct monthly evacuation drills and two shelter-in-place drills per year. Evidence: 1. There were no emergency evacuation drills conducted in November or December 2018 and April or May 2019. 2. The record indicates that the last SIP drill documented was in December 2017.

Description: Based on a review of nine staff files the facility failed to conduct all required background checks on all staff. Evidence: 1. There are no central registry clearances on file for staff #4 (hired 4-30-19), #6 (hired 5-1-19), #8 (hired 1-2-19).
