Comments:

A complaint investigation was initiated on April 29, 2019 with another local agency and ended on May 29, 2018 regarding a five year old child and inappropriate behavior guidance. Interviews were conducted and documentation was reviewed. There was not a preponderance of evidence to support the allegation regarding behavior guidance; however, non-related violations were found. The non-related violations are identified in the Violation Notice.

Plan of Correction:	Center Management will work closely with NVFS Director of Volunteer Opportunities to ensure that all regular volunteers have documentation of completed TB screening.

Description:	Based on classroom observation, the center did not ensure that all hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: On 4/29/2019, Classroom #202 had an unlocked teacher cabinet with spray bottles containing bleach spray solution.
Plan of Correction:	Center Management will review Health and Safety guidelines regarding hazardous and toxic substance storage and exposure to children.

Description:	Based on classroom observation, a classroom did not ensure that all electrical outlets had protective covers that are of a size that cannot be swallowed by children. Evidence: On 4/29/2019, Classroom #202 had electrical outlets without protective covers by the teacher's desk and by the computer table.
Plan of Correction:	Center Management will provide each classroom with the appropriate sized outlet covers for electrical outlets.
