An announced monitoring inspection was conducted with the director and staff. There were 65 children in care with a age range of six weeks to five years old. Children were observed during rest time, free play and snack. Eight children and six staff records were reviewed, All required paper work was posted. Standard #: 22VAC40-185-70-A Description: Based on review of six staff records not all had documentation of staff's education. Evidence: Staff A with a start date of 5/28/18 Did not have any documentation of education in the record. Plan of Correction: Copy of GED requested.
Standard #: 22VAC40-185-370-5 Description: Based on review of infant logs and staff interviews staff are not offering children that cant roll over 30 minutes of tummy time. Evidence: The staff stated they only offer three minutes of tummy time to children that cant roll over Plan of Correction: Correct tummy time is being given and documented properly.
Standard #: 22VAC40-185-510-A Description: Based on review of children's medication not all had a parents Authorization. Evidence: Child A did not have a parents authorization for an Epi-Pen, Benadryl and allergy relief medication Plan of Correction: Proper medication forms are completed and done.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on review of six staff records not all employees that have lived out side of the state of Virginia in the past five years had an out of state search of central registry check with in 45 days of employment. Evidence: Staff A with a start date of 5/29/2018 lived in the state of NC with in the past five years did not have an search of central registry from North Carolina. Plan of Correction: North Carolina Central Registry sent in.
