Comments:
An unannounced monitoring inspection was conducted with the director and staff. 50 children were in care during the inspection and the age range of the children was six months to eleven years old. Children were observed during free play. Staff should offer the children in-depth activities and roll model positive behaviors for the children in care. Five staff are current in MAT. All staff are current in First Aid and CPR. Five staff and children's records were reviewed. Standard #: 22VAC40-665-610-A
Description: Based on observation not all area of the center were maintained in a clean, safe and operable condition. Evidence: The carpets in the Toddler room ( the pond room) and two's room ( the garden) were dirty and stained. There was large areas of standing water on the playground and under the slides.

Plan of Correction: We will have the carpets cleaned. We have called a company that will be adding dirt and the water will be diverted to the street.

Standard #: 22VAC40-665-620-A
Description: Based on observation not all hazardous substances were kept locked. Evidence: There was Lysol spray stored in an unlocked cabniet assessbile to the children.
