Comments:
An inspection took place on 06/17/2019 between the hours of 9:45am and 2:30pm. During this time, there were 30 children in care. The youngest child was 3 years old and the oldest child was 12 years old. The children were observed swimming, eating lunch, engaging in free play, and napping. Also observed were swim safety practices, playground structures and safety, sanitation and cleanliness, staff and children's records, drill records, and first aid kit contents.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on observation and staff interview, the provider failed to have each staff member submit documentation of a negative tuberculosis screening no later than 21 days after employment of volunteering. Evidence: 1. Staff number (5) failed to submit documentation of a negative tuberculosis screening and the staff member's first day of employment was 5/13/2019.

Plan of Correction: The director has sent the staff person today for a TB screening/test and will ensure that all staff have TB as required.

Standard #: 22VAC40-185-540-C
Description: Based on observation and interview with staff, the provider failed to maintain minimum required contents of the first aid kit. Evidence: 1. The first aid on the bottom floor did not include triangular bandages and a first aid manual.

Plan of Correction: One triangular bandages and manual was found and placed in the kit and another triangular bandage will be ordered today.

Standard #: 22VAC40-185-540-E
Description: Based on observation and interview with staff, the provider failed to possess a battery operated flashlight. Evidence: 1. There was no flashlight located in the lower level classrooms. Staff stated that she did not possess a flashlight.

Plan of Correction: The director placed a flash light in the kit during the inspection.
