Comments:
On 6/19/19, a review of staff personnel files was conducted on this date by inspectors Margaret Allworth, Mahrukh Aziz, Leah Pagala, and Derek Acosta at the Pennino SACC office building. Record review of substitutes are documented with Silverbrook site file.

Violations:
Standard #: 22VAC40-185-240-C
Description: Based on record review the center did not ensure all staff members completed 16 hours of staff development training related to child safety and development and the function of the center. Evidence: Staff #1 record has documentation of 14 hours of staff development training between June 2018 and June 2019.

Plan of Correction: Documentation of training hours will be updated to reflect training hours received.
