An unannounced focused inspection of the center's school age building was completed on 6/21/19 from 11:30am until 12:35pm. At the time of entrance, there were 85 children in care, including 34 school age children, with 12 staff present. A sample of 4 children's records and 2 staff records were reviewed. Children were observed eating lunch and engaging in free play. Required postings and the first aid kit in the school age building were reviewed. Two violations in the areas of administration and special care provisions and emergencies are identified on the violation notice and were discussed with the center director during the exit interview.
Standard #: 22VAC40-185-60-A Description: Based on record review and interview, the center did not ensure that each child's record contains all the required information. Evidence: 1. The record for child 1 does not contain the name, home address and home phone number for the father. 2. The center director confirmed that child 1's record does not contain the required information for the child's father. Plan of Correction: The center responded with the following: The missing child information was requested the same day as the visit. Moving forward management will ensure during paperwork appointments that all required information is filled in. DM will review quarterly for compliance.
Standard #: 22VAC40-185-550-E Description: Based on a review of the emergency drill log and interview, it was determined the center did not maintain a record of the dates of the practice drills for one year. Evidence: 1. There was no emergency evacuation drill documented for May 2019 in the school-age building. 2. Staff 1 stated that an evacuation drill was completed in May 2019 but was not documented in the drill log. Plan of Correction: The center responded with the following: The drill logs for the SA annex will be updated and maintained on or before 6/30/19. Moving forward, management will verify that during all drills separate records will be maintained for the annex. DM will review quarterly for compliance.
