Comments:
The licensing inspector conducted an unannounced inspection on Tuesday, June 4, 2019 in response to a self-reported incident that the Department received on May 3, 2019 regarding supervision. The inspector was on site at the center on June 4, 2019 from 10:30am to 12:30pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The inspector completed additional interviews on June 13, 2019. The preponderance of the evidence gathered during the investigation validates the self-reported incident of inadequate supervision. Violations were cited regarding supervision and center policies. See the violation notice for more details. Violations:
Standard #: 22VAC40-185-40-E
Description: Based on documentation review and interviews, the center did not maintain compliance with their own policies and procedures required by standards. Evidence: The center's policies and procedures regarding supervision were not followed on May 2, 2019 when Teacher A did not have child #1 and child #2 within sight and sound supervision and discovered that the children were in the restroom alone. The center?s policies and procedures for supervision of children states that "All individual and or groups of children must be supervised at all times while in our care", "Children must not be left alone or unsupervised in classrooms, hallways, offices, lobbies, bathrooms, playgrounds, or vehicles." and "Closely monitor bathrooms when used by more than one child at a time. Be aware of children entering and exiting restrooms. Children should not use restrooms or toilet stalls in groups, nor should they linger in restroom areas for more than a few minutes." These policies and procedures were not followed during the above described incident on May 2, 2019.

Plan of Correction: - All Staff will be retrained on Child Supervision Records and KCE policies on supervision and transitions. - Management will have acknowledgment forms signed and placed in staff files for licensing and District leader to view during visits. - Management will complete 2 signatures daily on the back of CSR's along with times completed to ensure that accuracy of attendance is being recorded. - Management will complete observation of large group transitions and coach staff on any corrections needed. - Regional Program quality advisor will visit within 90 days to ensure that all pieces are being completed. - Administrative actions will be taken if any staff continue to not follow our supervision standards. - District leader will do unannounced visits to the center to ensure that staff are being retrained and that observations are completed. - District leader will make sure any new management is trained [in] CSR and supervision practices. - Program Quality Business Partner will complete a 90 day follow up with teachers and management to ensure that implementation of classroom management activities are taking place.

Description: Based on observation and interviews, the center did not ensure that children under 10 years of age were always within actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom. Evidence: 1. While taking attendance, Teacher #1 was notified by a student that both child #1 and child #2 were in the restroom alone. 2. Teacher #1 and the center's representative both stated that Teacher #1 was sitting at a table near the restroom. 3. The center's representative reported to the inspector that it is hard to hear children in the restroom while sitting in the location where Teacher #1 was sitting due to the classroom volume, location of the restroom, and restroom door material. 4. Teacher #1 stated that child #1 and child #2 was not seen going into the restroom and that they were not aware of the two children's location until another child informed them. 5. Child #1 and child #2 were both under the age of 10.

Plan of Correction: - Work orders will be placed to have a technician come evaluate the rest room doors and any other alternatives we can use. - Teachers and Center management will review the licensing standards regarding supervision within sight and sound. - Additional resources will be given to staff to help with classroom management and systems support supervision in older age groups. - Management will continue to have conversations with staff around the importance of compliance and supervision safety. - All new staff coming in will observed supervision and transition activities in classrooms prior to being in ratio. - District leader will do unannounced visits to the center to ensure that staff are being retrained and that observations are completed. - District leader will make sure any new management is trained [in] CSR and supervision practices. - Program Quality Business Partner will complete a 90 day follow up with teachers and management to ensure that implementation of classroom management activities are taking place.
