Comments:
An unannounced monitoring inspection was conducted from 11:30am-1:45pm. There were 108 children in the direct care and supervision of 24 staff members. The additional rooms/spaces being used for the SACC summer program were measured and inspected. A sample of 10 children's records, 5 staff records, 6 injury reports, 6 medications, the physical plant, and emergency procedures were reviewed. The children were observed eating lunch, going outside, and participating in a variety of activities. If you have any questions, please contact me at shahana.green@dss.virginia.gov .

Standard #: 22VAC40-185-510-A
Description: Based on a review of children's records, the center did not obtain parental authorization to administer medication for 1 out of 6 medications. Evidence: 1. Child #11 has an emergency medication on site, however, there was no medication authorization form on file for the medication.

Plan of Correction: We will ask the parents to fill out the required form.

Standard #: 22VAC40-185-550-M
Description: Based on a review of injury reports, 3 out of 6 injury reports were incomplete. Evidence: 1. 3 out of 6 injury reports were missing 1 of 2 required signatures.

Plan of Correction: Corrected during inspection.
