Violations:
Standard #: 22VAC40-185-280-D
Description: Based on observations and discussion with the staff, the facility failed to ensure that cleaning and sanitizing materials shall not be located above food, food equipment, utensils or single-service articles and shall be stored in areas physically separate from food. Evidence: The locked storage closet in the Fireflies Classroom contained 2 aerosol cans of disinfectant and a plastic spray bottle of Bleach and Water on a shelf directly above 2 packages of baby wipes, napkins and latex gloves.

Plan of Correction: The single-service items were relocated during the inspection. The staff will store the cleaning products away from the single-service articles. The staff will be informed of this as well.

Standard #: 22VAC40-185-550-D
Description: Based on document review, the facility failed to implement a monthly practice evacuation drill. Evidence: The drill log did not have a monthly practice evacuation drill documented for the month of April 2019; a shelter-in-place drill was conducted in April 2019 but there was no documentation of a monthly practice drill taking place.

Plan of Correction: The monthly drills will be conducted.

Standard #: 22VAC40-185-560-G
Description: Based on observations, the facility failed to ensure that when food is brought from home that the food container shall be clearly dated and labeled in a way that identifies the owner. Evidence: 1. There were 4 lunch boxes observed in the school-aged storage area without a date and not labeled in a way that identifies the owner. 2. There was a bottle of Sunny D in the red cubbie system by the front door that was not dated nor labeled to identify the owner.

Plan of Correction: The closing staff will be instructed to gather all leftover lunch boxes at the end of the day for parents to get the next morning.
