Reminder: Conduct at least two shelter-in-place drills and one lock down drill annually.

Comments:
The licensing inspector conducted an unannounced monitoring inspection from 9:00am - 11:45pm. There were twenty two children in care: four infants with two staff, five toddlers with one staff and thirteen preschool/school-age children with two staff. The children were observed playing with blocks, listening to a story and playing on the playground. Five staff's records and five children's records were reviewed. Standard #: 22VAC40-185-140-A
Description: Based on record reviews, the center failed to obtain documentation of a physical examination within thirty days of enrollment. Evidence: 1. The inspector reviewed five children's records. One of five records reviewed (child record #1) did not contain documentation of a physical examination and child's date of enrollment was 05/25/2019.

Plan of Correction: The director will request a copy of the child's immunization documentation from the child's parent and maintain in the child's record.

Standard #: 22VAC40-185-60-A
Description: Based on record reviews, the center failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed five children's records. One of five records reviewed (child record #1) did not contain street addresses for the listed emergency contacts. One of five records reviewed (child record #2) did not contain proof of identity and age and child's date of enrollment was 04/09/2019.

Plan of Correction: The director will request this information from the children's parents and monitor the children's records to ensure all information is complete.
