Comments:
An unannounced Subsidy Health and Safety (SHSI) Inspection was conducted on July 9, 2019, from approximately 1:50 pm to 5:10 pm. There were 56 children with five staff present and additional staff on site for administrative support. Children were observed playing small group games. The sample size of records reviewed included six children's records and five staff records. Four medications were reviewed. Areas of non-compliance were noted on the violation notice. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover the Subsidy Health and Safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. The exit interview was conducted with the center director. Standard #: 22VAC40-665-520-B-11
Description: Based on vendor interviews, the vendor did not ensure there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The record for child #5 confirms, child #5 has a diagnosed food allergy (nuts) and there was not a written allergy care plan. This was verified by the center director.

Plan of Correction: The center director will contact each parent to submit an allergy plan from their local physician.

Standard #: 22VAC40-665-550-2
Description: Based on vendor interviews, the vendor did not ensure to inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional. Evidence: One injury report reviewed dated 6-21-2019 documents that child #6 sustained an injury, fracture of growth plate, right ankle while in care at the center. Child #6 sought medical treatment from a medical professional. The departments representative was not notified of this injury as required.

Plan of Correction: The center director will utilize the on line form provided by VDSS and backup the form and send it the licensing agent.
