Comments:

The licensing inspector conducted an unannounced complaint inspection on June 28, 2019 in response to a complaint received by our department on June 17, 2019. The inspector was on site from 8:50 AM to 10:45 AM. The allegations of the complaint were inadequate staff-to-child ratios, inadequate supervision, and forbidden actions. Classroom observations were made, interviews were conducted, and records were reviewed. The preponderance of the evidence gathered during the investigation supports one of the allegations and the complaint is determined valid. See the violation notice for more detail. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to the licensing office within 5 business days. Your plan of correction should contain 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description:	Based on observation, the center did not maintain all areas in a clean, safe and operable condition. Evidence: During the inspection, the licensing inspector observed multiple items of trash and multiple broken toys on the playground.
Plan of Correction:	Per administration, staff will clean the playground after every use. The director will check the playground at the end of the day. Each lead teacher will be responsible for cleaning.

Description:	Based on observation, the center did not ensure that children under ten years of age shall always be within actual sight and sound supervision of staff. Evidence: 1. During the inspection, the licensing inspector observed one staff member supervising the school age class and the Pre-K class from a shared doorway. While the staff member supervised the Pre-K class during a restroom break, the school age class was not within sight of a staff member. 2. During the inspection, the licensing inspector observed two instances of children leaving the school age class to use the restroom located in the hallway without notifying staff.
Plan of Correction:	Per administration, the director will hire another staff to be a back up caregiver when teachers call in. Teacher are responsible for call out 24 hours before shift start.
