Comments:
An unannounced renewal inspection was conducted on 7/10/19 from 11:05am until 1:10pm. At the time of the tour, there were 70 children in care with 10 staff present. A sample of 7 children's records and 7 staff records were reviewed. Children were observed participating in morning learning activities, eating lunch, and transitioning to nap time. Hand washing, restroom, infant feeding and diapering and field trip procedures were also observed. Medication, first aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills, children's injury reports and center postings were reviewed. One violation in the area of administration is identified on the violation notice and was discussed with the center director during the exit interview.

Description: Based on record review and interview, the center did not ensure that documentation of additional immunizations were obtained once every six months for children under the age of two years. Evidence: 1. The most recent immunization record for child 1, age 1 year, is dated 10/11/18. Child 1 has an enrollment date of 10/22/18 and an updated immunization record was due in April 2019. 2. The center director confirmed that an updated immunization record for child 1 was not obtained by the center.

Plan of Correction: The center responded with the following: This was corrected on 7/12/19. Going forward management will update immunization spreadsheet monthly to ensure continued compliance.
