Description: Based on review of three staff files, and an interview with the center director, the facility failed to have documentation that each staff meets the qualifications for the position hired. Evidence: Staff #2 and staff #3 did not have documentation of the education or the experience required to be a program leader and both staff were observed alone with a group of children.

Plan of Correction: Documentation of qualifications will be placed in each staff's file.

Description: Based on review of one medication, the center did not have written authorization to administer medication to a child in care. Evidence: The center did not have a medication authorization form signed by a physician and parent for Child #5's medication.

Plan of Correction: Medication was returned to parent today.
