An unannounced monitoring inspection was conducted from 9:00 - 11:00 am with the center director and staff. There were 13 children upon the LI's arrival, ranging from nine months to 11 years of age, present with 3 staff supervising. Children were observed creating an edible arts and crafts project ("ant hills"), participating in structured circle time activities, listening to stories read by a volunteer grandmother and eating lunch, which is brought from home. Five children's records were reviewed and in compliance. Three staff records were reviewed. Required information was posted. The last fire drill was conducted: 7/10/19 and the last shelter-in-place drill was conducted: 5/20/19. One medication was reviewed; the director has current Medication Administration Training (MAT) certification. Standard #: 22VAC40-185-70-A Description: Based on review of three staff files, and an interview with the center director, the facility failed to have documentation that each staff meets the qualifications for the position hired. Evidence: Staff #2 and staff #3 did not have documentation of the education or the experience required to be a program leader and both staff were observed alone with a group of children. Plan of Correction: Documentation of qualifications will be placed in each staff's file.
Standard #: 22VAC40-185-510-G Description: Based on review of one medication, the center did not have written authorization to administer medication to a child in care. Evidence: The center did not have a medication authorization form signed by a physician and parent for Child #5's medication. Plan of Correction: Medication was returned to parent today.
