Technical Assistance:
Recommended that management speak with parent that has a doctors note authorizing the use of a long term medication that has expired. Staff are not trained in determining if medication has colored. Parent should update the medication, the Inspector does not believe that here is a shortage of this medication anymore.

Comments:
An unannounced renewal inspection was conducted from 9:00 am-1:00. There were a total of 133 children with 28 staff members present. Children were observed during snack time, engaged in group activities learning about flowers and vegetables, a special trip to the ice cream truck and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children records, the center has not requested at least annually parent confirmation that the required information in the child's record is up to date. Evidence: Children records have not been updated annually. Child #1 last update was 3/12/15, Child #2 last update was 12/29/14 and child #3 last update was 7/9/14.

Plan of Correction: We will pick one week each year to have staff request annual updates from parents. Our first update will be in September.

Standard #: 22VAC40-185-510-G
Description: Based on a review of children medications, the center has not ensured that medication on the prescription label is in accordance with the prescriber's instructions pertaining to dosage, frequency, and manner of administration. Evidence: The prescription label for Child #4's medication is 0.15 mg. The medication authorization form signed by the physician on 8/27/18 says to administer 0.3 mg.

Plan of Correction: We will ask the parent to have the physician complete a new form with the correct dosage.
