Violations:
Standard #: 22VAC40-185-40-E
Description: Based on staff interviews and documentation review and the observation of the center's video recording system, staff failed to follow the center's own policies and procedures required by the Standards. Evidence: 1. The center's Face to Name policy for accounting for children at all times requires staff to compare the children's faces to the name listed on the Face to Name Transition and Attendance sheet and document the exact time of the Face to Name attendance check and the transition code for the new location such as PG=playground. 2. On 6/4/19, Staff #1 failed to complete the required Face to Name documentation as the children transitioned from the Beginner's class to the playground. 3. Staff #1 left Child #1(2yrs) alone in the classroom for approximately 1 minute until Staff #2 informed Staff #1 that one child was missing. 4. On 6/5/19, Staff #3 failed to complete the Face to Name documentation as the Intermediate Class transitioned at a gate threshold leading from one playground to another playground.

Plan of Correction: Staff 1: The staff member noted in the Systematic Deficiency Repeat Violation 22VAC40-185(2)40-E resigned their position with Nobel Learning Communities. Therefore, they left their full-time position and are no longer affiliated with the company. Staff #2 was retrained on June 2019 with a Plan of Correction. She was retrained on Playground Safety, Student Transitions, Forbidden Actions, Licensing Information, Face to Name and Mandated Reporter. (COMPLETED 06/17/2019) A Mandatory Staff meeting with the Principal was held on June 2019. All Staff were retrained (THIS INCLUDED STAFF 2 & 3) on proper procedures and protocol when signing students in and out on the transition sheets and documenting in Tadpoles. How to properly communicate with the office when changes occur due to scheduling/emergencies was also reviewed. A school meeting on the extreme importance of following the proper steps was held by the Principal in which hands-on-practice (Role Playing) and review was implemented. Training on the use of the walkie-talkies was also presented for improved communication. The Principal/Management/Support Team is responsible for performing routine checks and providing continual training for all staff on the proper use of transition sheets and walkie- talkies to ensure that students always remain in sight and sound of their classroom teacher. (TEACHERS ARE NOT PERMITTED TO GO OUTSIDE OR RETURN INSIDE WITHOUT A MANAGEMENT/SUPPORT TRAINED TEAM MEMBER BEING PRESENT TO SIGN OFF ON THE FACE TO NAME TO COMPLETE THE TRANSITION AND TO ENSURE THAT ALL CHILDREN ARE ACCOUNTED FOR AND IN ATTENDANCE). Staff were retrained on Mandated Reporter, Supervision Policies , Student Transitions and Outside Time. All trainings listed will be on going to constantly remind our Staff of correct policies and procedures. Trainings will all be documented with meeting notes on file along with staff names in attendance.

Description: Based on staff interviews, review of staff written statements and observation of the center's video recording system, one child was not always within actual sight and sound supervision of staff. Evidence: l. On 6/4/19, Child #1 (2yrs) was left alone for approximately 1 minute in the Beginner's Class after the child's class transitioned outside to the playground. 2. Staff were unaware that Child #1 was left alone in the classroom. Staff #2 stated "I realized one was missing when [Staff #1] came outside." Staff #2 further stated "I told [Staff #1] that we were missing one child." 3. Staff #1 was recorded returning to the Beginner's classroom and found Child #1 alone in the classroom.
