An unannounced non-mandated monitoring inspection, as a result of a self-reported incident, was conducted on 07/09/2019. Record reviews, document reviews and interviews were conducted during the inspections; some interviews were conducted along with local agencies representatives. Standard #: 22VAC40-185-340-F Description: Based on observations and discussion with staff, the facility failed to ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff. Evidence: 1. Staff #1 had a total of 10 children, ages 3 years-4 years, in care in Building #2. 2. Staff #1 stated that she took Child #1 to the Boys Bathroom to have a talk with Child #1. 3. Staff #1 stated that during the conversation with Child #1 she crouched down with her right arm resting on the door frame and that she entered the Boys Bathroom to get Child #1 a tissue following their conversation. 4. The remaining 9 children in the care of Staff #1 were seated at tables in the dining area of Building #2; Staff #1 was the only staff member in Building #2. 5. During the time that Staff #1 was at the door frame and inside of the Boys Bathroom the other 9 children seated in the dining are were not within actual sight and sound supervision of staff. Plan of Correction: Reemphasized to all staff that children have to be in sight and sound supervision at all times.
Standard #: 22VAC40-185-410-1 Description: Based on document review and interviews, the facility failed to ensure that the threat of physical punishment, striking a child, roughly handling or shaking a child shall be forbidden. Evidence: 1. In a written statement to staff administration, Staff #1 wrote " I reminded Child #1 that his mother gave me permission to spank his butt". 2. During an interview on 07/09/2019, Staff #1 stated that she had threatened Child #1 with calling his dad and "popping his butt". Plan of Correction: Reemphasized to all staff forbidden actions concerning the children.
Standard #: 22VAC40-185-550-M Description: Based on discussion with staff, the facility failed to maintain a written record of children's serious and minor injuries in which entries are made the day of the occurrence and shall include all of the elements as required by the standards. Evidence: Staff #1observed a scratch on Child #1's neck while in care on 07/02/2019, the scratch was not observed upon arrival to the program on 07/02/2019. Staff #1 did not complete a written record of the child's injury. Plan of Correction: Reemphasized to all staff to write an injury report for every accident.
