Comments:
An announced initial inspection was conducted on 07/22/2019. The owner/program director was present during the inspection. One applicant record was reviewed during this inspection. The physical plant was inspected and measured to verify capacity. The emergency preparedness plans, parent handbook, staff handbook, and emergency supplies were reviewed at this time. The inspector discussed the following with the staff: room measurements/capacity, playground equipment/resilient surfacing, areas and equipment, nutrition and subsidy. The inspector arrived at 8:40 a.m. and departed at 12:45 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Violations:
Standard #: 22VAC40-191-40-C-1-A
Description: Based on record review and discussion with the applicant, the applicant failed to obtain a search of the central registry upon application for licensure. Evidence: The findings from a search of the central registry were not available for review during the scheduled initial inspection. The search request has been sent but the results have not been received.

Plan of Correction: A second request was sent via certified mail on 07/19/2019. Phone calls to the Office of Background Investigations were made during the inspection to get updates on the request. Verification of the results will be sent to the Licensing Inspector upon receipt.

Standard #: 63.2(17)-1721.1-B-2
Description: Based on record review and discussion with the staff, the facility failed to obtain a fingerprint eligibility letters for all applicants/agents listed on the application. Evidence: Applicant #1 did not have a fingerprint eligibility letter for review during the scheduled initial inspection. Applicant #1 stated she was under the impression that licensing provided the required Fieldprint code necessary to obtain to a fingerprint search.

Plan of Correction: Applicant #1 contacted the necessary entity to obtain a Fieldprint code and is expecting to have that code within 2 days and will schedule an appointment for fingerprinting at that time; the Fieldprint account has been set up. The results will be sent to the Licensing Inspector at that time.
