Technical Assistance:
Suggested the use of the Division's model forms to record emergency drills.

An unannounced monitoring inspection was conducted today. There were 11 children in care, with a total of 6 staff members assigned to their ongoing direct supervision. Children's records, the physical plant, programming, children's nutrition, emergency preparedness items, new staff records, background checks, and children's medications were inspected today. The children were observed playing with toys, engaging in creative play, taking an outdoor walk, and eating lunch from home. Pleasant, friendly, and welcoming interactions between staff and children were observed throughout the course of this inspection. Description: Based on a review of staff records, it was determined that one record was not complete. Evidence: Staff record C does not include documentation of 2 completed reference checks.

Plan of Correction: The reference checks will be completed and documentation placed in the file.

Standard #: 22VAC40-185-430-G
Description: Based on observations, it was determined that one piece of indoor climbing equipment has not been placed over resilient surfacing as required. Evidence: In the open play room, there is an indoor climbing piece. Although the specific climbing portions are lower than 36inches, there is a bar across the top that could be accessed and climbed on by the children. The height of this section of the climbing piece is 49inches. This piece of equipment has been placed over carpeting, but not over acceptable resilient surfacing.

Plan of Correction: We will place surfacing mats under the piece of equipment.

Standard #: 22VAC40-185-510-G
Description: Based on a review of children's medications, it was determined that one authorization form does not match the prescription dosage instructions. Evidence: The dosage instruction on one child's long-term prescription medication authorization does not match the dosage instructions indicated on the medication prescription label.

Plan of Correction: The parent will be asked to have the form corrected and re-signed by the physician.

Standard #: 22VAC40-185-550-D
Description: Based on a review of emergency drill documentation, it was determined that the school did not practice a monthly evacuation drill. Evidence: The school did not practice an evacuation drill during the month of May, 2019.

Plan of Correction: We will do a drill every month.
