A complaint investigation was conducted regarding an allegation of lack of care, protection, and guidance, lack of supervision, and staff-to-child ratio. The investigation was conducted with the director beginning 11:45 AM and ending 1:41 PM on June 14, 2019. Staff interviews were also conducted on June 18, 2019. Four staff interviews were conducted. Three staff records were reviewed. Three staff records and two children's records were reviewed. Incident/Accident reports were reviewed. The allegations regarding care, protection, and guidance and lack of supervision were unable to be substantiated due to insufficient corroborating evidence. The allegation regarding staff-to-child ratio was determined to be valid. Standard #: 22VAC40-185-350-E-2 Complaint related: Yes Description: Based on staff interviews, the facility did not maintain one staff member for every five children ages 16 months through 2 years old. Evidence: Staff members stated that on June 11, 2019 at approximately 2:00 PM there was one staff member caring for ten children in the toddler room for approximately 15 minutes. Plan of Correction: On this day, a staff member returned from lunch later than usual. In the future a member of management will step in if needed until both caregivers return to the classroom by 2pm.
