Comments:
A subsidy health and safety inspection was completed on July 22, 2019 from approximately 10:15a.m. to 1:15p.m. to review supplemental health and safety requirements. Thirty children were in care and eight staff were on premises during the inspection. Staff and children were observed in classrooms and on the playground engaged in activities to include activities for infants, free outside play, lunch time, hand washing, and nap time. Interviews and interactions were conducted and medications were reviewed. Eight children?s records, five staff records and one corporate member record was reviewed. The Owner/Director was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Description: Based on a review of staff records, the licensee failed to ensure that within the first 90 days of employment or subsidy vendor approval all staff who work directly with children shall complete Virginia Preservice Training for Child Care Staff for one of five staff. Evidence: 1. Staff #3's record, employed on April 5, 2019, lacked completion of the Virginia Preservice Training as required. 2. Administrator #1 acknowledged this staff has not completed the training as required.

Plan of Correction: The Director/Owner has notified staff that the training must be completed by the end of August 2019.

Standard #: 22VAC40-665-580-E-1
Description: Based on a review of staff records, the licensee failed to ensure that within the first 90 days of employment or subsidy vendor approval all staff who work directly with children shall obtain current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care for one of five staff. Evidence: 1. Staff #3's record, employed on April 5, 2019, lacked completion of CPR training. 2. Administrator #1 acknowledged this staff has not completed CPR training as required.

Plan of Correction: The Director/Owner indicates a CPR training is scheduled for August 10, 2019 that this staff will attend.

Description: Based on a review of staff records, the licensee failed to ensure that within the first 90 days of employment or subsidy vendor approval all staff who work directly with children shall obtain current certification in first aid for one of five staff. Evidence: 1. Staff #3's record, employed on April 5, 2019, lacked completion of first aid training. 2. Administrator #1 acknowledged this staff has not completed first aid training as required.

Plan of Correction: The Director/Owner indicates a CPR training is scheduled for August 10, 2019 that this staff will attend.

Description: Based on observation of records, the licensee failed to develop a written policy and procedure that describes how each group of children receives care by consistent staff or team of staff members. Evidence: 1. There were no written consistent care procedures to review. 2. Administrator #1 indicates that while the center does practice and implement consistent care, procedures have not been written.

Plan of Correction: The Director/Owner will write a consistent care policy.

Standard #: 22VAC40-665-770-B-5
Description: Based on a review of records, the licensee failed to include a component in the emergency preparedness plan regarding continuity of operations procedures to ensure that essential functions are maintained during an emergency. Evidence: 1. The emergency preparedness plan lacked a component as to how continuity of operations procedures to ensure essential functions are maintained during an emergency will occur. 2. Administrator #1 acknowledged this has not been included in the emergency preparedness plan.

Plan of Correction: The Director/Owner will add this component to the emergency preparedness plan.
