Discussed children records need to have parents' employers address.

Licensing inspector arrived at 10:15 am and departed at 12:40 pm to conduct a licensing inspection and a Subsidy Health and Safety inspection. At the time of the tour there were 82 children in care with 13 staff . Additional staff arrived during the inspection. Children were observed working on developing gross motor skills, tablet activity , variety of stations, tummy time in the infant classroom and movie day in school age class. The sample size consisted of 8 children records and 7 staff records. Areas of non-compliance was reviewed with the director.

Standard #: 22VAC40-665-540-A Description: Based on record review and staff interview, the vendor did not ensure documentation of the tuberculosis (TB) is completed with in the last 30 calender days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department. Evidence: Staff #1 date of employment 7/15/2019 the tuberculosis (TB) results were dated 02/08/2019. Staff #2 date of employment 4/05/2019 the tuberculosis (TB) results were dated 04/05/2019. Staff #4 date of employment 4/25/2019 the tuberculosis (TB) results were dated 05/13/2019. Staff #7 date of employment 5/31/2019 the tuberculosis (TB) results were dated 06/25/2019. Staff #1, Staff#2, Staff#4 and Staff #6 TB test results were not completed within the last 30 calendar days of the date of employment. Plan of Correction: Before hiring we will send TB requirements to potential employees.
