Comments:
An unannounced, mandated renewal inspection was conducted on 7/25/2019. Ninety-four children were in care. Morning activities, lunch service and preparation for nap time were observed. Medication administration and injury records were reviewed. A sample size of five children's records were reviewed. Fourteen staff records and six administrator/board member records were reviewed at the central office on 7/26/2019. Exit meetings were conducted with the program director and administrator prior to closure of the inspections.

Description: Based upon review of fourteen staff records, the facility has not ensured that documentation of a repeat tuberculosis screening is obtained for all staff at least every two years from the date of the initial screening. Evidence: The most recent tuberculosis screenings for the staff 1, 3, 5, 6, 7, 8 and 10 were obtained in June 2017.

Plan of Correction: The facility responded with the following: All staff will be required to submit documentation of tuberculosis screenings at least every two years.

Description: Based upon program director interview, the facility did not ensure that there was documentation to show that the catering service is approved by the local health department. Evidence: The program director was unable to identify the catering service being used or to provide documentation of local health department approval of the catering service.

Plan of Correction: The facility responded with the following: Documentation of local health department approval of the catering service being used was obtained prior to closure of the inspection.
