63.2(17) License & Registration Procedures

Comments:
An unannounced monitoring inspection was conducted from 10:15 am - 12:00 noon with center staff. There were 15 children, ranging from eleven months to five years of age, present with 3 staff supervising. Children were observed napping, playing outside and eating snack. Snack served today apples and almonds. Five children's records were reviewed and in compliance. Five staff records were reviewed. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a review of five staff records, it was determined that not all staff records were complete. Evidence: The records for staff #2 and staff #3 did not have written information to demonstrate that the individual possesses the education and experience required by the job position.

Plan of Correction: All staff records include written information that they have ample qualifications to be in the job positions they are in.

Standard #: 22VAC40-185-80-A
Description: Based on a review of the written attendance record and an interview with staff, it was determined that the facility did not ensure all children were included on the written attendance record. Evidence: The attendance record for the Children's House listed 12 children in attendance however only 11 children were present in the class.

Plan of Correction: In the Children's House there were 12 children who were in attendance. One child had gone on a field trip. This was known by all.

Standard #: 22VAC40-185-320-B
Description: Based on observation, and an interview with staff, it was determined that not all restroom areas were equipped with soap, toilet paper, and disposable towels within reach of children. Evidence: There were no paper towels within reach of the children in the toddler classroom bathroom. A roll of paper towels was on a high shelf and three cloth towels were hanging by the sink.

Plan of Correction: All bathrooms have soap, toilet paper and paper towels for use by all.

Standard #: 22VAC40-185-340-D
Description: Based on observation, and an interview with staff, the facility failed to ensure that each grouping of children has at least one staff member who meets the qualifications of program leader regularly present. Evidence: Staff #2 was alone with one infant when the LI arrived and stated that the program leader comes in the afternoon.

Plan of Correction: All staff members who are program leaders have documentation to this on file.

Standard #: 22VAC40-185-340-F
Description: Based on observation, the facility failed to have children under the age of 10 within actual sight and sound supervision of staff. Evidence: When the LI arrived at the facility at 10:15 am, and rang the doorbell, Staff #2 left the infant classroom to answer the door. When the LI asked to speak to the person in charge, the staff member walked towards the stair well to call for assistance and/or ring a bell. The children were out of staff sight and sound supervision for less than 2 minutes.

Plan of Correction: This staff member was trained again that she may not leave to answer the door no matter who it is. Communication for staff support should have been implemented as trained.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on a review of five staff records, it was determined that no Out of State Child Abuse and Neglect Search was requested by the end of the 30th day of employment. Evidence: Staff #3 (date of hire: 1/15/19), who indicated that they have lived out of the state of Virginia in the past five years, had no documentation of an out of state search request in the file.

Plan of Correction: We have heard from the state of CA that they do not do this. Email is in file as instructed.
