Comments:
An unannounced monitoring inspection was conducted from 1:15pm-3:15pm. There were 19 children in the direct care and supervision of 4 staff members. A sample size of 4 children's records, 3 staff records, evacuation drills, emergency materials, and the physical plant were reviewed. The new (temporary) daycare space was inspected since the staff/children have been relocated to a room/space upstairs due to a broken A/C unit on the lower level. The children were observed taking a nap. After the children woke up, they were observed eating a snack consisting of goldfish, milk, and a cupcake to celebrate a child's birthday and then they had story time. Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records, 1 out of 3 staff records reviewed was not complete. Evidence: 1. Documentation of 2 of more references for staff #3 (doh: 5/16/2019) was not available during the inspection.

Plan of Correction: We will locate the references or contact new ones.

Standard #: 22VAC40-185-290-3
Description: Based on observation, all electrical outlets did not have protective covers on them. Evidence: 1. 2 outlets underneath the table where the attendance binder is located did not have protective covers on them. 2. 1 outlet under the window, next to the A/C unit, did not have a protective cover on it. 3. 1 outlet behind the white cabinet did not have a protective cover on it.

Plan of Correction: This was corrected during the inspection.
