Technical Assistance:
Discussed the automated Central Registry search results. Findings are now emailed to the requester. Central Registry contact: 804.726.7549 Please call to obtain clarification regarding the results of your one staff. Discussed the definition of fall zones as described in "National Program for Playground Safety's "Selecting Playground Surface Materials: Selecting the Best Surface Material for Your Playground," February 2004, and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use.

Comments:
An unannounced Monitoring inspection was conducted this date. A total of 16 children with 4 staff present today. When the Inspector arrived, children were busy with the morning activities. A nice transition to AM snack was observed. Staff ensured children's hands before eating. Staff were seen positively engaging with the children. Walk through of the Center was made to include the outdoor playground. Positive interaction was observed between staff and children. The center was found well maintained, clean and equipped with ample supply of equipment. No medications on site today. No topical ointments. A total of 5 children's records and 2 staff records were reviewed today. Previous violations from the inspection in January were reviewed, and corrective actions were verified. Standard #: 22VAC40-185-90--A
Description: Based on review of children's records, the written agreement for two records are not complete. Evidence: Child #1 and #3's records do not verification that parents were provided a parent handbook which includes additional information such as sick policy and notification of communicable diseases.

Plan of Correction: We will have the parents sign the Parent Handbook disclosure.

Standard #: 22VAC40-185-280-E
Description: Based on observation of the classrooms, cleaning material and an insecticide was not stored separately from each other. Evidence: In the first Classroom from the main hall, a bleach sanitizer was found inside the same cabinet with an insecticide.

Plan of Correction: We will store these items separately.

Standard #: 22VAC40-185-330-B
Description: Based on observation of the outside playground, resilient surfacing was not maintained. Evidence: The mulch located at the bottom of the fall zones on the playground was not maintained in such a manner that it complies with minimum safety standards of a depth of six inches.

Plan of Correction: We will order additional mulch for the playground.

Standard #: 22VAC40-185-330-E
Description: Based on observation of the outdoor play area, two sandboxes were not maintained as required. Evidence: The two sandboxes were not in use on the outdoor playground. Both sandboxes were found not covered.

Plan of Correction: We will ensure that sandboxes are covered daily when children are not playing in them.

Standard #: 22VAC40-185-560-G
Description: Based on observation of the lunch boxes brought from home, lunch boxes were not properly labeled. Evidence: All lunch boxes were dated with today's date, however some lunch boxes were found that did not include the child's name.

Plan of Correction: We will ensure that all lunch boxes are labeled with both name and date.

Standard #: 22VAC40-191-60-C-2
Description: Based on staff record review, a completed Central Registry finding was not found for one staff. Evidence: Staff #3 was hired on 6/1/19 based on documented start date. A Central Registry search request was requested on 5/31/19. No documentation found of the search finding.

Plan of Correction: We will contact the Central Registry today to inquire about the status of the search request.
