Comments:
An unannounced monitoring inspection was conducted today from approximately 9:45am to 11:00am. 30 children were directly supervised by 5 staff members. The director was in a classroom, as the 6th staff member, to provide assistance with ratios. The physical space, staff and children's records, attendance, emergency evacuation drills, required posted documents and medications were reviewed. Medication policies were in compliance. The health inspector arrived during the inspection. Children were observed playing outside on the playground, transitioning from outside to free play, engaged in dramatic play and interacting with staff members. Interactions were positive. The center continues to provide developmentally appropriate materials and activities. Areas of non-compliance are identified in the Violation Notice. For questions, please contact stephanie.salvatori@dss.virginia.gov.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, equipment in the center was not kept in a safe manner. Evidence: A mirror on the slanted ceiling, in the upstairs classroom, is not secure and flush. 7 mirror holders with screws are used to secure the mirror but only 2 are flush against the ceiling. A gap of approximately 1/4 inch allows the mirror to hang from the ceiling, which could possibly be a hazard to fall in the classroom.

Plan of Correction: We will contact maintenance and have it repaired right away.
