Technical Assistance:
When items are placed on the diapering table there is a risk of a transfer of germs from the diapering table to the object. Suggest removing vines growing around the playground fence before they become accessible to children. Discussed different methods to document "action to prevent reoccurrence of injury" on injury forms.

Comments:
An unannounced renewal inspection was conducted on 07/31/2019 from 10:24am - 1:25pm. At the time of entrance there were 67 children in care with 11 teaching staff and two administrative staff. Children were observed during morning program time, which included outdoor play, lunch and nap. Records were reviewed for eight children and six staff. Violations were observed in two parts of the CDC standards and are listed on the violation notice. The violations were reviewed with the program director at the conclusion of the inspection.

Standard #: 22VAC40-185-60-A
Description: Based on record review and interview, the center failed to ensure that children's records contain all of the required elements. Evidence: 1. The enrollment record for child 1 lacked a work phone number for both parents. a. The enrollment record indicated that both parents were employed outside of the home. 2. Administrative staff confirmed that a work telephone number was not available for each parent.

Plan of Correction: This was completed on 7/31/2019. Paperwork appointments are required to ensure that all paperwork is 100% complete and reviewed by 2 managers prior to family enrolling. Management will conduct periodic reviews to ensure continued compliance.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center failed to ensure that the diapering surface shall be used only for diapering or cleaning children. Evidence: 1. After completion of a diaper change at 10:50am , in the toddler classroom, the staff person placed a tablet and diapering log directly on the diapering table.

Plan of Correction: Diapering Policy and training will be reviewed with appropriate staff by 8/2/19. The safety captain and management will do periodic inspections to ensure compliance.
