Comments:
An unannounced monitoring inspection was conducted today. There were 31 children in care, with a total of 8 staff members assigned to their direct supervision. Children's records, new staff records, staff background checks, staff training documentation, children's medications, classroom programming, emergency preparedness items, and the physical plant were inspected today. The children were observed enjoying outdoor water play activities, singing songs, engaging in circle time discussions, listening to stories, and eating lunch from home. This inspection was completed between approximately 10:30am and 1:00pm. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observations, it was determined that one area of the playground has not been maintained in a safe condition as required. Evidence: Upon entering the playground, the Inspector observed a utility ladder placed against the fence that encloses the toddler playground. This ladder was in the area used for open play by the older children, and was accessible to the children.

Plan of Correction: The ladder was immediately moved inside the fenced area that is not used by children.

Standard #: 22VAC40-185-330-B
Description: Based on observations, it was determined that there is not enough resilient surfacing in the fall zone areas on the playground. Evidence: There is not enough mulch in the fall zone areas of the preschool playground. In the fall zone areas, the mulch has worn down to dirt.

Plan of Correction: We are already scheduled to get more mulch later this month. I'm also looking into other options of products to use for resilient surfacing.

Standard #: 22VAC40-185-500-A
Description: Based on observations, it was determined that in one classroom the children's hands were not washed immediately prior to eating lunch. Evidence: In the Preschool classroom, the children were observed washing their hands for lunch then sitting on the floor for circle time before eating. The children did not wash their hands immediately prior to eating.

Plan of Correction: The schedule is off today due to water play. The children usually have circle time, then wash hands to eat. That schedule will be maintained each day.

Standard #: 22VAC40-185-510-C
Description: Based on a review of children's medications, it was determined that one authorization form has expired. Evidence: In the Infant classroom, one child's medication for an over-the-counter medication expired on 5/30/19.

Plan of Correction: The medication is no longer needed and will be sent home with the parent.

Standard #: 22VAC40-185-510-J
Description: Based on a review of children's medications, it was determined that two medications were not locked. Evidence: In the Toddler classroom, two children's medications were stored inside a zipper compartment in a staff backpack. This backpack was hanging on a high hook on the inside of the classroom door. Neither the zipper compartment nor the backpack were locked.

Plan of Correction: The medications will be moved to a locking bag.
