Comments:
An unannounced monitoring inspection was conducted today from 11:45am-2:30pm. There were 220 children (3yrs-12yrs.) directly supervised by 60 staff. The physical plant, 10 staff records, 10 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, having swimming lessons, participating in various activities to include sports class, team games, pool time, reading time, crafts, arts, playing outside, and eating lunch. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review, each staff member did not have tuberculosis test/screening documentation within 21 days after employment. Evidence: Staff#4's record does not contain TB test/screening documentation. The staff member was hired on 6/13/2019.

Plan of Correction: We'll have the staff member set up an appointment and get it done.

Standard #: 22VAC40-185-550-D
Description: Based on record review and interview, documentation of evacuation drills was found to be incomplete. Evidence: Documentation of required monthly evacuation drills were not on file for July 2019.

Plan of Correction: We'll check with the director of facilities to see if the drill was documented.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, the center did not obtain a central registry finding for each staff member within 30 days of employment. Evidence: The records of Staff#1 (DOH: 06/12/2019), #2 (DOH:06/16/2019), #6 (DOH:06/08/2019), #9 (DOH:06/11/2016), and #10 (DOH: 06/08/2019) did not contain documentation that central registry check results were obtained.

Plan of Correction: The forms were submitted we need to check to see if we've received the results.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review, the center did not obtain fingerprint based background check results for staff members prior to their first day of employment. Evidence: Staff#3's record (DOH: 06/16/2019) does not contain the fingerprint based background check result required prior to their first day of employment. Staff#4's record (DOH: 06/13/2019) does not contain the fingerprint based background check result required prior to their first day of employment.

Plan of Correction: I will double check if they were submitted, if not we'll have them redone within a week.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on staff record review, there is no documentation that Child Abuse and Neglect checks were initiated for employees that have resided outside the state of Virginia within the last 5 years. Evidence: Staff#1 (DOH: 06/12/2019), #3 (DOH:06/16/2019), #6 (DOH:06/08/2019), and #9 (DOH:06/11/2016) have resided outside the state of Virginia within the last 5 years, their records did not have documentation that out-of-state child abuse and neglect results were obtained.

Plan of Correction: Moving forward the forms will be submitted as needed.
