Comments:
An unannounced monitoring inspection was conducted on 8/5/19 from 10:15am - 11:45am. During the inspection there were nine children ages three months old through ten years old in care with two staff. Children were observed participating in various activities in the classrooms. Records were reviewed for five children and two staff. There was no medication at the facility. Emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation and interviews, it was determined that the facility did not ensure that all areas and equipment of the center shall be maintained in a safe and operable condition. Evidence: 1. The white wooden fence that separates the two outdoor play areas has areas where the wood is broken and splintering. There also exposed nails along this fence. 2. Staff #2 confirmed the fence was not in a safe and operable condition.

Plan of Correction: Not available online. Contact Inspector for more information.

Standard #: 22VAC40-185-470-A
Description: Based on observation and interview, it was determined that the licensee did not ensure that when wading pools are located on the premises of the center, the pool is enclosed by a safety fence and gates which are in compliance with the applicable edition of the Virginia USBC (13VAC5-62) and shall be kept locked when the pool is not in use. Evidence: 1. There was an above ground pool that was in excess of two feet deep that was located on the large outdoor play area that was not enclosed by a safety fence or locked. 2. Staff #2 confirmed the pool was not enclosed by a safety fence or locked.

Plan of Correction: The facility responded: The pool will be removed and not used any more.

Standard #: 22VAC40-185-570-C
Description: Based on a review of documentation and interview, it was determined that the licensee did not ensure that the record of each child shall contain the child's feeding schedule. Evidence: 1. There was not a record with the brand of formula for each child who is on formula. 2. Staff #2 confirmed that there was not a record with the brand of formula for each child who is on formula.

Plan of Correction: The facility responded: I added the names and or designation of formula and breastmilk to the infants files, as well as, their schedules posted in the infant room. There are two formulas listed for two of the children because their emergency supply cans are different from what they bring for daily use
