Technical Assistance:
Discussed resilient surfacing and documentation of fire inspection.

Comments:
Licensing inspector arrived at 10:30am and departed at 1:15 pm to conduct a licensing inspection . At the time of arrival was 63 children in care with 12 staff . Additional staff arrived during the inspection. The Licensing Inspector observed children arriving, leaving for field trip, freely playing and outside play. The sample size consisted of 6 children records and 6 staff records. Areas of non-compliance were reviewed with the director.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review , the licensee did not ensure screening was submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering. Evidence: Staff #1 employment date May 2, 2019 the most recent T.B. test was 02/28/2018. The T.B. test was more than a year old.

Plan of Correction: Management has been retrained on managing first day paperwork and requirements for all new staff. Moving forward, all new hires will have a TB test result that is less than 30 days old. The DM will review all new hire files monthly for compliance.

Standard #: 22VAC40-185-260-A
Description: Based on record review, the licensee did not ensure a annual fire inspection was completed. Evidence: The most recent fire inspection was June 15, 2018.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the licensee did not ensure all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: Observed the following inside and outside the building: - In the two year old bathroom, there was a toilet with a lid that was not fitted and was able to be moved and possibly tipped over . - Outside on the preschool playground area, playground composite on the right side there was yellow peeling paint.

Plan of Correction: We will work in conjunction with our maintenance company to ensure that both of these items are corrected on or before 8/23.19. Our safety captain will ensure that these items are managed on their daily safety checklists to ensure compliance.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the licensee did not ensure there hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: The staff bathroom was unlocked there was a bottle of of bleach and water and a sanitizer accessible to children.

Plan of Correction: Staff will be retrained on managing the locked staff bathroom at our August staff meeting. Management and the Safety Captain will observe daily for compliance.

Standard #: 22VAC40-185-510-N
Description: Based on record review, the licensee did not ensure when an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. Evidence: Child #7 medication authorization form expired 3/17/2019. Medication was not picked up by parent with in 14 days. Medication was in medication box on August 1, 2019.

Plan of Correction: Management will ensure that all expired medications will be sent home each Friday. If not, they will be disposed of immediately. DM will observe for compliance quarterly.
