Comments:
An unannounced monitoring inspection was conducted today from 12:00pm-2:10pm. There were 81 children (infants-5yrs.) directly supervised by 11 staff. The physical plant, 5 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, preparing for lunch, eating lunch, cleaning up and getting ready for nap time. Hand-washing procedures were observed. There was a supply of materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-70-A
Description: Based on records reviewed, the center failed to maintain complete staff records. Evidence: 1) Staff 1 is missing two references. 2) Staff 2 is missing date of hire and documentation of orientation. 3) Staff 3 is missing date of hire. 4) Staff 4 is missing date of hire, documentation of two references checked, and documentation of orientation. 5) Staff 5 is missing date of hire.

Plan of Correction: We will work on completing all staff files.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment inside of the center were not kept in a safe and operable condition. Evidence: 1) In the preschool room, there was a broken toy high chair. The broken piece causes a potential safety hazard to children. 2) In the preschool room and the toddler room, there were areas of peeling paint. In the school age classroom, there was an area of peeling paint in the block area. The peeling paint causes a potential safety hazard to children.

Plan of Correction: A work order was placed for the peeling paint previously. The toy high chair was removed to be repaired.

Standard #: 22VAC40-185-560-K
Description: Based on observation, tables were not cleaned after a meal. Evidence: In the school age camp classroom, there were pears and other spilled liquids on the tables from lunch. The school age camp was lining up to leave the classroom and the tables were not cleaned.

Plan of Correction: We will remind all staff tables need to be cleaned after each meal.
