Comments:

A Subsidy Health and Safety inspection was conducted on 7/26/19 from 11:15am - 1:55pm. During the inspection there were 36 children ages four months old through eight years old in care with seven staff. Children were observed participating in various activities in the classrooms, playing outside, and eating lunch. Records were reviewed for four children and five staff during the inspection. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements, were discussed during the exit interview, and are identified on the violation notice.

Standard #:	22VAC40-665-500-E
Description:	Based on interviews and observation, it was determined that the vendor did not ensure to post, with parental approval, or keep in a way that is immediately accessible to staff, a current list of all children?s allergies, sensitivities, and dietary restrictions. Evidence: 1. There was not a list of all children?s allergies, sensitivities and dietary restrictions for the children enrolled at the facility in the Infant classroom. 2. Staff #5 confirmed that there was not a list of all children?s allergies, sensitivities and dietary restrictions for the children enrolled at the facility in the Infant classroom.
Plan of Correction:	The facility responded: An ?Allergies? section was placed in the information binder in the infant classroom; all teachers were advised of the document.
Standard #:	22VAC40-665-540-A
Description:	Based on a review of five staff records, it was determined that the vendor did not ensure that staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department. Evidence: 1. The record for staff #3 (date of hire 7/8/19) contain documentation of a TB screening that was dated 5/13/19. 2. Staff #6 Program Director)confirmed that staff #6 had not completed a TB screening within the last 30 calendar days of the date of employment.
Plan of Correction:	The facility responded: Staff #3 was instructed to complete a more recent TB screening.
Standard #:	22VAC40-665-650-E
Description:	Based on a review of policies and procedures for the facility and interview, it was determined that the vendor did not ensure that a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members was developed and implemented. Evidence: 1. The Licensing Inspector could not find the policy and procedures for maintaining consistent staff at the facility during the inspection. 2. Staff #6 (Program Director) confirmed that there was no documentation available for viewing during the inspection regarding how the vendor will ensure that each group of children receives care by consistent staff or team of staff members.
Plan of Correction:	The facility responded: A policy was created for ?Maintaining Consistent Staff? and placed in the Emergency Action Plan binder.
Standard #:	22VAC40-665-710-I
Description:	Based on observation and interview, it was determined that the vendor did not ensure that filled comforters were not used by children less than 12 months of age while sleeping. Evidence: 1. A filled comforter was observed in a crib in the Infant room. 2. Staff #5 confirmed that there was a filled comforter being used for a child less than 12 months of age in the Infant classroom.
Plan of Correction:	The facility responded: All filled comforters (or similar) were removed from the infant area completely.
