Comments:
An unannounced renewal inspection was conducted today from 10:10am-1:00pm. There were 49 children (infants-5yrs.) supervised by 11 direct care staff. The physical plant, 3 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed reading books, playing with musical instruments, having circletime, and playing outside on the playground. Toileting and hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Areas of non-compliance are identified in the Violation Notice. Description: Based on observation, 1 of 2 playground areas did not protect children from hazards. Evidence: The large outdoor playground area had an open gate leading to the side of the building that had utility boxes and 2 cans of paint.

Plan of Correction: Teachers will be re-trained and reminded to check and close the gate.

Standard #: 22VAC40-185-350-E-3
Description: Based on observation, the 2-year old classroom did not ensure that there was one staff member for every eight children. Evidence: From approximately 11:00am to 11:05am, the 2 year old classroom had 9 children and 1 staff member.

Plan of Correction: Ratios were corrected immediately. Teachers will be retrained on supervision requirements.
