Comments:
An unannounced monitoring inspection was conducted on 08/07/2019 from 10:55am - 1:10pm. At the time of entrance there were 41 children in care with 5 teaching staff and the program director. The children ranged in age from two years - school age. Children were observed during late morning program time and lunch. Records reviewed for one new staff and eight children in care. Medication and emergency supplies were additionally reviewed. One violation was noted and is listed on the violation notice.

Description: Based on observation, the center failed to ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99. Evidence: 1. One area of mulch, to the left of the small plastic play structure, is overgrown with grass. a. Overgrowth of grass causes the mulch to lose the ability to provide cushioning.

Plan of Correction: The child care board will be notified of this immediately. This area of mulch will be addressed within two weeks.
