Comments:
An unannounced Complaint inspection was investigated this date. Allegations of staff supervision were investigated. Staff interviews were conducted, policies and procedures reviewed and observation of classrooms were made. The preponderance of evidence does not support a valid finding. However, during the investigation other violations were found that were not part of the allegations. Thank you for your cooperation today. Complaint related: No
Description: Based on review of the Center's roll call sheets and staff statements, staff failed to follow their own policies and procedures for identifying children at all times. Evidence: Based on the roll call sheet in the Nature Center classroom, the call sheet had 9 children documented presently in the classroom, however 7 children were actually present. According to Staff #1, two children had left earlier in the day and the time of departure was not documented. The roll call sheet for Think Tank had Child #1 present in the classroom during nap time. According to Staff #1, Child #1 was moved from the Think Tank class to the Nature Center class for nap time.

Plan of Correction: All staff will be retrained regarding the use of the roll call sheet and ensure documentation is completed to account for every child and their location.

Complaint related: No
Description: Based on observation of the School Age classroom, 12 inches of space was not maintained between two children resting. Evidence: Licensing Inspector observed two children laying side by side sharing a blanket while resting.

Plan of Correction: We will reschedule their activities throughout the day to include quiet activities. We were no longer have rest time.

Complaint related: No
Description: Based on observation of the School Age classroom, the areas used for the designated rest period did not include all required items. Evidence: One child who was observed resting in the School Age classroom was laying directly on the classroom floor with an area rug used as a cover.

Plan of Correction: We will reschedule their activities throughout the day to include quiet activities. We were no longer have rest time.
