The Licensing Inspector conducted an investigation in response to a self report incident as a result of a child found unsupervised in the hall bathroom on July 19, 2019. Interviews were conducted with staff and staff and child record reviewed. It was determined that the child was walking with the Preschool Summer camp class up the stairway and left the group. The teacher reported it to the office immediately. The child was not within sight and sound supervision for approximately 1-2 minutes. The information gathered during this investigation supports the allegations; violations were discussed with the center director. If you have any questions you may contact me at Stacy.Doyle@dss.virginia.gov or 571-835-0386.

Standard #: 22VAC40-185-340-F Description: Based on multiple staff interviews, children under 10 years of age were not always within actual sight and sound supervision of staff. Evidence: 1.
