An unannounced monitoring inspection was conducted on August 13, 2019 from approximately 9:15 am to 12:00 pm. Eleven staff were present with seventy-two children. Additional staff were on site for administrative duties. Seven children and seven staff records were reviewed.

Standard #: 22VAC40-185-280-B Description: Based on observation, facility did not ensure hazardous chemicals are kept locked. Evidence: Multiple chemicals to include bleach, disinfectant solutions and paint were observed in an unlocked storage closet in the front hallway. The chemicals were labeled with caution/danger and keep out of reach of children. Plan of Correction: Closet will be kept locked.
Standard #: 22VAC40-185-330-B Description: Based on observation, facility did not ensure there is adequate resilient surfacing in the fall zones under and around moving equipment on the playground. Evidence: A four-seater teeter totter was observed on the preschool playground. There was 2' of resilient surfacing from the edge of the equipment to a concrete pad when there should be at least 6' of resilient surfacing around all moving equipment. Plan of Correction: One arm of the teeter totter will be removed. A maintenance request was completed during inspection. The piece of equipment will be deemed "out of order" now until the repairs are made.
