Technical Assistance:
Consultation was given on staff completing the injury form with a time the parent was notified rather than "at pickup" to ensure concise documentation.

Comments:
An unannounced monitoring visit was conducted today from 11:20-12:30. The Co-op program was not observed as it finishes at noon. Twenty two children were present with four staff and two counselors in training. No medication was reported to be on-site during the inspection. The theme was camping and the kids were making "Smores" out of craft sticks, reading with teachers and transitioning to the gym before the day ended. Reviewed a sample of children's files, emergency preparation, physical space and staff records. Areas of non-compliance are identified in the violation notice. Description: Based on record review, 2 of 4 children's records are incomplete. Evidence: Child #2 is missing two emergency contact names and addresses and Child #3 is missing one emergency contact address.

Plan of Correction: Center staff will request information from the parent.
